Amputee Player Registration

If you are an amputee, friend or family of an amputee, or an interested party, and you would like an amputee to receive information on our annual event (Amputee Golf Tournament and Sponsor/Amputee Scramble), please complete the following online registration.

Once added to our player database, the registered amputee will receive a package each March with detailed event information.
