2008 Event (Amputee Tournament Players Championship)

Amputee Golf Charities has provided multiple methods for an Amputee Golfer to enter the 2008 Event. Choose the one that is the most convenient.

One method of entering the 2008 Event is to download the Entry Form. After downloading - print the form, fill-in, and mail to Amputee Golf Charities (address is on form).

Enter Online:

To enter the 2008 Event online, please fill in the following fields then select the "Submit Entry" button at the bottom. Your online entry will be automatically mailed to Amputee Golf Charities. A member of the Tournament Committee will take your information, enter you in the tournament, and send you an acknowledgement (by email, postcard, or phone).
