Amputee Golf Charities has provided multiple methods for an Amputee Golfer to enter the 2012 Event. Choose the one that is the most convenient.

One method of entering the 2012 Event is to download the Entry Form. After downloading - print the form, fill-in, and mail to Amputee Golf Charities (address is on form).
